In what way does PHCS provide services for people with disability and their families?

PHCS has been involved in providing support in the home for people with disability and their primary carers since the inception of the Home and Community Care (HACC) program in the mid 80s. Services may involve domestic assistance, personal care, respite or social support. For more information on this program see the HACC section of the website.
In more recent years PHCS has been involved in the management of individualised funding for people receiving Intensive Family Support (IFS)funding or Accommodation Support Funding (ASF)from the Disability Services Commission. In 2004 there are approximately 30 children and adults being supported in this way.

What models of service does PHCS provide for people with disability who have individualised funding from the Disability Service Commission?

There are two main models provided by PHCS. These are
a)Visiting support - Support Workers visit the home of the person with disability and provide assistance in the way agreed and for the hours agreed in the service plan. This plan is developed in conjunction with all key stakeholders. The level of support can be quite high in terms of hours per week (eg 80 hours per week) and in the complex care tasks undertaken (eg hoist transfers, bowel care, peg feeding). Individuals and family can elect to be involved in the selection of Support Workers.
b) Alternative Family Care - in this model the person with disability lives in the home of an alternative carer who provides him/her with a stable and positive environment in an essentially normal family home. A tax free Carer Reimbursement is paid to acknowledge the additional support and supervision required. Regular weekend and annual respite for the alternative family carer is included in every option.

Are there models of service that PHCS does not offer?

Perth Home Care Services does not have residential accommodation such as group homes and does not provide services that involve long term coverage by shift workers.

Are there any criteria in terms of type of disability for eligibility for service?
|
|
|
Perth Home Care Services caters for people with all types of disability and all ages.
|

What geographical areas does the agency cover?
|
|
|
Most of the agency's services are in the Perth metropolitan area. The country areas in which individual options are managed include Northam and surrounding areas, Geraldton and Carnarvon.

If I am in the process of making an application to DSC through the Combined Application Process (CAP) can your agency sponsor the application?

Perth Home Care Services is able to assist with preparation of the application if the type of service you are seeking is within our scope. See the paragraph above about the models of service. If you are already in receipt of services from PHCS we will be in a better position to prepare the application. However if you are not known to PHCS we can still assist given an adequate period of time to understand your circumstances.
