Perth Home Care Services Inc provides services to people who are frail aged and younger people with disabilities. Our services assist people to remain living in their own homes and aim to maximise the quality of life and health and wellbeing of our clients, their carers and their families. AssessmentDuring assessment you have the opportunity to tell us about your needs. Assessment includes a full explanation of the services available to you. Wherever possible you will be provided with choices about the type, frequency and nature of services provided to you. If we are unable to assist we will provide you with information about alternative services. Case ManagementCase management refers to the assistance received by a client with complex care needs from one of our program coordinators. This person may coordinate planning and delivery of services from more than one agency. Crisis CareThe Crisis Care Support Service offers short-term emergency support for the main carers of people who are frail aged or have a disability in the event of:
This service is specifically for those incidents that occur without warning and result in the main carer being unable to continue to care for their family member. Domestic AssistanceDomestic Assistance is normally provided in the home and includes services such as dishwashing, house cleaning, clothes washing, shopping and bill paying. Nursing ConsultancyServices include: Case management. Training sessions and competency testing of Support Workers with nursing type procedures, assessment of complex clients including incontinence issues, manual handling problems, wound care. Personal CarePersonal Care is normally provided in the home and includes helping the client with daily self care tasks such as eating, showering or bathing, and grooming. Perth Home Care Services employees do not administer drugs but we include monitoring and prompting for medication. RespiteThis service is designed to support the Primary Carer (the person who normally looks after the aged or disabled person) so that they can have relief from their caring role and pursue other activities or interests. One of our support workers will come to the residence and stay with the client while the Primary Carer takes a break. Respite can be a few hours or involve an overnight stay or other live in support. Social SupportSocial Support is normally provided in the client’s home but may include accompanying the client on an excursion or trip. The support is provided to them as an individual and helps participate in society. It includes keeping them company, helping them do paper work, taking them shopping, banking or to attend an appointment.
