Community Aged Care Packages are planned and coordinated services to help people with complex care needs remain living in their home. Each package is tailored to an individual's unique circumstances and needs, with the aim of maximising independence and enhancing daily living skills.

The services are flexible and can change as people’s circumstances alter. They take into account the support of family and friends. The CACP's facilitate connection to the local community.
Perth Home Care Services currently has 57 CACP's funded by the Commonwealth Department of Health and Ageing. There are 30 allocated to the East Metro region and 27 to the North Metro region.
