Part Time

Are you in tune with what is happening in the local community?

Perth Home Care Services are seeking Mental Health Support Workers (part time) to work alongside the people we support to maintain and improve their quality of life and independence within the community.

You will work with people in their local communities supported by a strong team environment. Most importantly, you will become part of a team which is dedicated to improving the lives of community members with a mental illness.

Our Mental Health Program focus is a recovery promoting environment, identifying and building on the strengths of the people we support, working in partnership to establish supports that meet people's individual needs.

• a Certificate IV in Mental Health or studying towards a degree in Social Work, Occupational Therapy, Nursing

Ventilator Dependant Quadriplegic (VDQ) Program - Toodyay Area
Support Workers
Part Time

We are looking for people in Toodyay to join our specialist VDQ team assisting Geoff with quadriplegia. He is dependent on a respiratory ventilator which enables him to live at home in Toodyay with his family. Support Worker (Carer) experience is not essential as PHCS provide paid extensive training to join the team.

• Excellent pay rates are offered with penalty remuneration
• Part-time flexible shifts are available
• Weekend and overnight's available

CATCH – VDQ Program Perth Metro Area
Community Support Workers – Specialised Skills
Part Time

Both CATCH and VDQ Programs were established in 2006. The objective of each Program is to provide quality of life at home for people who, previously, would have required long-term hospitalization. In the move from hospital environment to community and family, PHCS is able to offer individuals and their families high levels of on-going clinical care and support.

The VDQ Program (Ventilator Dependent Quadriplegic – Community Care Program) provides assistance to non-compensable persons requiring mechanical ventilation, who wish to re-establish and maintain their lives in a community setting.

The CATCH Program assists children dependent on technology to return to their family home, once medically stable. Support is offered to families to care for their child in the home. The children in CATCH all have a long term dependence on medical technology to maintain respiratory function.

The role of support workers in the CATCH and VDQ Programs involves working with people and their families on a one-to-one basis to assist their participation in positive family and community interaction. It includes performing clinical tasks such as tracheostomy care, oxygen management, and ventilator management. Successful applicants will be provided on-the-job training by the primary carer (family) or existing team members and will also undertake Clinical Theory sessions with a PHCS Nurse Consultant. Once deemed competent by the Nurse Consultant, applicants are able to work as a part of the specialist team supporting the client at home.

We welcome applicants with prior experience or Certificate III in Community Care (Disability or Aged) but are also keen to hear from anyone who thinks they would have the drive and commitment necessary to make a positive difference in peoples’ lives. A current driver’s licence is essential, plus access to a reliable vehicle.

Support Worker Positions – Client Specific

Chris Needs Support Workers

Be paid to train on our VDQ Program

In 2002 local Broome man Chris Fong had an accident that left him with C4/C5 quadriplegia. In 2007, after years in Royal Perth Rehabilitation Hospital and at The Quad Centre in Perth, Chris came home to Broome to live. He is supported by the Ventilator Dependent Program, an initiative of the Health Department, run by PHCS.

Chris is currently looking for more people to join the dedicated team of Support Workers who help him get the most out of life. Previous experience not essential as full on-the-job training is provided. A current manual driver's licence is necessary, plus willingness to work on a rotating roster. The 24/7 roster includes one overnight (2200-0700) and one afternoon shift (1200-2200) each week.

We are looking for Liaison Officer's to support and lead our field based teams

Do you think you would like to work for a company that makes a real difference to people's lives?

We are currently seeking individuals who are experienced in all aspects of personal care to take on a leadership/mentoring role for our Support Workers. You will lead by example to motivate and support the team in their daily duties.

Duties will include all levels of support, reviewing clients and buddying new employees. Ideally, the successful applicant will also have some basic clerical skills, together with good time management and organisational skills.

We are looking for individuals experienced in all aspects of personal care and who are skilful and dynamic to lead, develop and supervise support worker teams. Our Team Leader's will work in partnership with the people we support and their families, to ensure they have the support they need to live their lives.

In addition to providing direct support, the Team Leader will have duties including:

• Developing a good understanding of each person their team supports and what is important to them in their lives
• Developing positive relationships with people we support and their families
• Training, coaching and supervising support workers directly in their work
• Promoting and facilitating positive team work
• Organising staff rosters and schedules and providing back up support when required
• Developing potential resources and relationships with members of the community
